S/MC LITTLE LEAGUE

Registration fees for 2019 season:

$35 first child

$25 any additional child

Instructions:

• The Player Registration Form and Medical Release Form is required for each player

• First year players in our league will also need to hand in a copy of their birth certificate

• If you would like to help, volunteer or coach, please complete and return the Volunteer Form below.

• Parents, please review the Parent Code of Conduct for the SMCLL. We make every effort to create a safe, enjoyable and respectable season for the players, coaches and volunteers

Please make every effort to sign-up your player by February 01, 2019.

*For child online registration, please download the Player Registration Form and Medical Release Form to your computer. Then fill them out and save. Please digitally sign the forms or handwritten signature/scanned docs can be accepted.
